This must be filled out online by all associations even if the paper form or pdf were already submitted to Cathy. Sorry for the duplicate effort in that case. MAHA is really trying hard to streamline these procedures and make them less painful for all.
I believe that one of the things that you must verify when filling out the form is that you have completed the Association Contacts online form that was sent to association presidents. At this time (August 10) only about half the associations in the District have done so.
